Tulalip Contract Health Services

Mission:

The mission of the Tulalip Contract Health Services is to provide H.O.P.E. (Helping Our People Every Day) for quality specialty medical and dental care for Tulalip Tribal members living in Snohomish County.

EMERGENCY ROOM VISITS

You have 72 hours to notify CHS of an ER visit, you must call 360-716-5630 and leave a voicemail with your name, DOB, hospital and a number to reach you at.

PO REQUESTS

For PO requests you must call 360-716-5630 prior to your appointment, leave a voicemail with your name, DOB (date of birth), date of appointment, clinic where your appointment is scheduled at and a number to reach you at.

Eligibility:

1. Tribal members living in Snohomish County.
2. Other Natives living on the reservation.
3. A non-Native American pregnant woman with an eligible Native American living in Snohomish County during pregnancy and six weeks post-partum.
4. Services must be delivered within the United States.
5. All patients must verify their eligibility prior to appointments.

Registration Requirements:

1. Tribal verification (Tribal ID or certification of Indian blood)
2. Address verification (PUD/water/cable/cell phone bill, copy of lease, or letter from individual you are residing with)
3. Birth certificate
4. Social Security card
5. Insurance card
